Rent Assistance
To apply for rental assistance, you must provide the
following information from your landlord (on letterhead):

Address of the apartment where the client is living
·
Amount of monthly rent
·
Amount of rent past due rent or the amount of the deposit
·
Amount the client has paid toward the rent
·
Are the utilities included in the monthly rent?
·
Name, address and phone number where the assistance would be sent
·
In some cases additional information will need to be filled out by
the landlord
You will also need to provide:
·
Two pieces of identification
·
Proof of income (check stub or award letter)
·
You must be making regular rental payments
